How Do I Apply?

STEP 1:

Mag-login sa Online Application ng Kaiser. Just

to Login!

STEP 2: After niyan ay may lalabas na tab kung saan mag-fill out ka ulit ng detail katulad ng First Name, Middle Name, Last Name, Birthdate at Email. Ang agent name and code ay automatic na lalabas ang name ko.

(Click Create Quote after ma-fill out lahat)

STEP 3: After pindutin ng "Create Quote" ay lalabas ang result ng iyong proposal.

STEP 4: Once na-click mo ang compute quote ay mag-uupdate ang mga benefits ng Plan sa right side. Kung hindi ka satisfied sa pinakitang benefits ay maaari ka ulit pumili ng PLan, click mo lang ulit ang Plan Name and choose your desired plan. Again, click Compute Quote.

Choose Plan (K100 or 100,000 is just a plan name and the default) You can change it...​

But REMEMBER THIS 3 QUESTIONS BEFORE CHOOSING PLAN!

1. Magkano ang benefits na gusto mo? Milyon ba? Kapag gusto mo malaki, mataas na plan kunin mo.

2. Magkano ang income mo? Dapat depende din yan sa income mo. Kung gusto mo di baba lifestyle mo balang araw, dapat kung malaki income mo, lakihan mo din plan mo.

3. Magkano ang talagang naiipon mo? Magkano ang kaya mong masustain huhulugan?

STEP 5: Kapag naka-set na ang lahat ay pumunta sa lower portion at may makikita kang terms of payment like Monthly, Quarterly, Semi-annual, Annual and Spotcash

Choose Terms of Payment

Take note: Every year has 500 pesos policy fee.

NOTE: May discounts kapag Semi-annual, Annual and Spotcash. Sa annual ay 2 months ang discount at syempre mas malaki ang discount ng spotcash.

STEP 6: Once na nakapag-decide ka na kung anong Terms of Payment, just click CHOOSE.

STEP 7: Pag-click mo ng choose button may mag-appear na:

Just click the COMPLETE CLIENT APPLICATION NOW button or you can check your email to Complete your Kaiser Application Form.

STEP 8: Just fill out all the required information and proceed to the payment.

Pay your Kaiser Long Term Healthcare and send your payments thru Bank Deposits, Online bank Transfer or Remittance.

After mag-deposit sa bank account ay paki-picturan or i-screenshot ang proof of payment para ma-attached sa Kaiser Application Form.
